Elkhart Community Schools uses FinalForms for all middle and high school sports registrations. Whether you're new to FinalForms or returning for another season, follow the steps below to register your athlete for any sport in grades 6–12.

Step 1: Parent Registration

1. Visit FinalForms.
2. Click the Parent Icon, then select New Account.
3. Enter your name, date of birth, and email address, then click Register.
4. Check your email for a confirmation link. Click Confirm Your Account.
5. Create a password and confirm your account.
6. Click Register Student to begin adding your child.

Note: If you don’t receive the confirmation email within two minutes, check your spam folder. Still no email? Contact support@finalforms.com.

Registering Additional Students?

Log in, click My Students, and repeat the registration steps for each child.

Updating Info or Adding a Sport?

Log in anytime and click Update Forms to make changes for any student.

Step 2: Student Registration

1. Log in at FinalForms.
2. Click Login under the Parent Icon.
3. Click Add Student.
4. Enter your student’s legal name and other required details.
5. Select the sports your student plans to join, then click Update.
6. Complete and sign each form with your full legal name (e.g., Jonathan Smith), then click Submit Form.

Important: Beginning Fall 2025, families must upload the student’s IHSAA physical form in FinalForms. Paper copies will not be accepted. Contact the Athletic Office with any questions or technical difficulties.

Returning to FinalForms? Follow These Steps:

1. Go to FinalForms.
2. Click Login under the Parent Icon.
3. Review, update, and sign all required forms.
4. Select your student’s sports.
5. Your student will also receive an email with a link to re-sign forms as they did previously.

Need to register another child? Use the same account—no need to create a new one.
